I turned off assisted dialing. I cleared the cache and storage of the phone app. I rebooted my phone. Went back into settings for the phone. I activated hold for me. I activated Direct my call. I activated Call Screen. I then went to voicemail and turned off visual voicemail, then turned back on. VVM now works.

I spent at least 2 hours with T-Mobile and Google today and nobody said anything like "that's not supported", they all seemed to think it should work.
